Whilst there is value in learning from our mistakes, sharing best practices can reduce unnecessary failure.

Agile has been around in some shape or form for at least 40 years, yet we still hear many stories of woe where it has failed to deliver the goal of good working software. Why is that?

Well, many of the reasons are nothing to do with agile itself, and many other reasons are more to do with agile done badly than agile as a method in its own right.
